Abstract Objective：To evaluate the clinical characteristics and MRI diagnostic value of hirayama disease.Method：Analyse the clinical and MRI feature of hirayama disease in 6 cases proved by clinical.Result：5 males and 1 female in 6 cases，with muscle weakness and atrophy on uooer limbs， 5 cases showed unilateral and 1 case showed bilateral but asymmetric involvement.In the flexion position MRI scan，the forwords movement of lower cervical cord and
the abnormal lunular signal with posterior epidural space in all cases.Conclusion：Hirayama disease occurs mainly in young males.The hirayama disease shows characteristic manifestations on the flexion position MRI appearance，which can help the doctor make diagnosis in the early stage.
