Abstract Objective: To investigate the application value of small dense low density lipoprotein (sdLDL), low density lipoprotein (LDL) and thromboelastography (TEG) in the diagnosis of carotid atherosclerosis (CAS) in hyperlipidemia patients. Method: A total of 200 patients with hyperlipidemia who treated in Fengcheng Hospital of Traditional Chinese Medicine from November 2021 to October 2022 were retrospectively selected as the research subjects, and they were grouped according to whether accompanied by CAS. The patients with simple hyperlipidemia were divided into group A (n=94), and the patients with hyperlipidemia accompanied by CAS were divided into group B (n=106). In addition, those who were healthy and underwent physical examination at the same time in our hospital were selected as group C (n=50), and the relevant clinical data were collected from three groups. The sdLDL, LDL and TEG were compared among the three groups, and the value of sdLDL, LDL and TEG parameters in the diagnosis of hyperlipidemia CAS was analyzed. Result: The levels of sdLDL and LDL in group B were significantly higher than those in group A and C (P<0.05). The reaction time (R) and blood clot formation time (K) in group A and B were significantly shorter than those in group C, and the R in group B was significantly shorter than that in group A (P<0.05). The maximum amplitude (MA) and maximum tangent angle of elastic diagram (Angle) in group A and B were significantly higher than those in group C, and the MA in group B was significantly higher than that in group A (P<0.05). The results of ROC curve analysis showed that the area under the curve (AUC) values of sdLDL, LDL, R, and MA in the diagnosis of CAS respectively were 0.728, 0.667, 0.764, and 0.693, and the AUC value of CAS detected by the combination of the four measures was 0.832, which were significantly higher than that of the single index detection (Z=3.727, 4.850, 3.037, 4.386, P=0.000, 0.000, 0.002, 0.000), the sensitivity was 84.91%, and the specificity was 68.75%. Conclusion: sdLDL, LDL and TEG are closely related to CAS in patients with hyperlipidemia, and they all have certain diagnostic value, and the combined detection can significantly improve the diagnostic efficiency.
