Clinical Significance of Ultrasonography in the Diagnosis of Cervical Lymph Node Metastasis in Patients with Differentiated Thyroid Cancer |

Abstract Objective: To analyze the diagnostic value of ultrasonography in cervical lymph node metastasis in patients with differentiated thyroid carcinoma (DTC). Method: A total of 135 patients with DTC who received subtotal or total thyroidectomy in People's Hospital of Ruijin City from January 2017 to July 2022 were selected. All patients underwent ultrasonography before operation, and the results of postoperative pathological examination as the gold standard, the diagnostic efficacy of ultrasonography in cervical lymph node metastasis of patients with DTC was statistically analyzed. In addition, the ultrasonic image characteristics of patients with cervical lymph node metastasis and patients without metastasis were analyzed. Result: Among 135 DTC patients, 36 cases were found to have lymph node metastasis by pathological examination and 31 cases were found to have lymph node metastasis by ultrasonography. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of ultrasonography respectively was 80.56%, 97.98%, 93.33%, 93.55% and 93.27%. Kappa test showed that ultrasonography of lymph node metastasis in DTC patients has excellent consistency with the results of gold standard examination (Kappa=0.822, P=0.000). The proportions of slight calcification, long/short diameter of lymph nodes <2, cystic or liquefied areas, abundant blood flow of lymph nodes, disappearance of lymphatic hilum or unclear lymph node structure in metastatic group respectively was 83.33%, 80.56%, 69.44%, 86.11% and 72.22%, which were higher than 13.33%, 17.17%, 20.20%, 24.24%, 10.10% in non-metastatic group (P<0.05). Conclusion: Ultrasonography has a high diagnostic efficiency in the diagnosis of lymph node metastasis in patients with DTC, and there are great differences in the ultrasonic image characteristics between patients with lymph node metastasis and patients without lymph node metastasis.
