Effect of Laparoscopic and Choledochoscopic Surgery Combined with Yinchen Chaihu Decoction on Liver Function and Immune Function in Patients with Cholecystolithiasis and Choledocholithiasis |

Abstract Objective: To investigate the clinical effect of laparoscopic and choledochoscopic surgery combined with Yinchen Chaihu Decoction in the treatment of cholecystolithiasis combined with choledocholithiasis and its influence on liver function and immune function. Method: A total of 82 patients with cholecystolithiasis and choledocholithiasis admitted to Fengcheng Traditional Chinese Medicine Hospital from January 2020 to December 2021 were randomly divided into the control group and the observation group, with 41 patients in each group. The control group was treated with laparoscopic and choledochoscopic surgery, the observation group was treated with Yinchen Chaihu Decoction on the basis of the control group. The clinical efficacy, liver function indexes, immune function indexes and complications were evaluated and compared between the two groups 14 days after operation. The TCM syndrome scores were compared between the two groups 3 months after operation. Result: The total effective rate of the observation group was significantly higher than that of the control group, the difference was statistically significant (P<0.05). 14 days after surgery, the levels of GOT, TBA and GPT in the observation group were (57.63±5.49) U/L, (16.58±2.11) μmol/L, (52.96±5.42) U/L, which were lower than (65.47±5.76) U/L, (21.05±2.28) μmol/L, (60.24±6.19) U/L of the control group, the differences were statistically significant (P<0.05). 14 days after surgery, CD4+, CD4+/CD8+ in the observation group were (44.79±4.05)%, (1.67±0.21), which were higher than (40.31±3.88)%, (1.42±0.19) in the control group, CD8+ was (26.89±2.04)%, which was lower than (28.32±2.11)% in the control group, the differences were statistically significant (P<0.05). Three months after surgery, the scores of upper abdominal fullness, nausea and vomiting, abdominal pain and tenderness in the observation group were (0.65±0.11), (0.52±0.11), (0.71±0.12) and (0.68±0.12) points, which were lower than (0.87±0.14), (0.79±0.12), (0.89±0.14) and (0.82±0.13) points in the control group, the differences were statistically significant (P<0.05). The total incidence of complications in the observation group was lower than that in the control group, the difference was statistically significant (P<0.05). Conclusion: Laparoscopic and choledochoscopic surgery combined with Yinchen Chaihu Decoction can improve the curative effect of patients with cholecystolithiasis combined with choledocholithiasis, reduce the damage of liver function, accelerate the recovery of immune function, reduce the TCM syndrome score, reduce the occurrence of complications, and be safe and reliable.
