The Value of Multi-slice Spiral CT in the Diagnosis of Early Lung Cancer and Small Pulmonary Nodules

Abstract  Objective: To compare the signs and diagnostic value of multi-slice spiral CT (MSCT) in early lung cancer and small pulmonary nodules. Method: A total of 100 patients with small pulmonary nodules admitted to Yingtan 184 Hospital from January 2021 to December 2022 were selected as the research objects. All patients received MSCT examination and pathological examination, and the results of pathological examination as the gold standard, the MSCT signs of early lung cancer and small pulmonary nodules were compared, and the diagnostic value of MSCT for early lung cancer were analyzed. Result: Pathological examination results of 100 patients with small pulmonary nodules, 42 cases of early lung cancer and 58 cases of small pulmonary nodules; MSCT examination results, 45 cases of early lung cancer and 55 cases of small pulmonary nodules. MSCT signs: in early lung cancer, there were 29 cases (69.05%) with spicule sign, 31 cases (73.81%) with fuzzy boundary, 19 cases (45.24%) with angio-assemblage sign, 25 cases (59.52%) with pleural indentation sign, 26 cases (61.90%) with thorn sign and 25 cases (59.52%) with lobulation sign, however, in small pulmonary nodules, spicule sign, fuzzy boundary, angio-assemblage sign, pleural indentation sign and lobulation sign were relatively rare (P<0.05). The accuracy, sensitivity, specificity, positive predictive value and negative predictive value of MSCT in the diagnosis of early lung cancer respectively were 87.00% (87/100), 88.10% (37/42), 86.21% (50/58), 82.22% (37/45), 90.91% (50/55), and Kappa value was 0.725, and the consistency was good. The accuracy, sensitivity, specificity, positive predictive value and negative predictive value of MSCT in the diagnosis of early central lung cancer respectively were 85.71% (36/42), 76.92% (20/26), 100.00% (16/16), 100.00% (20/20) and 72.73% (20/26), and Kappa value was 0.709, and the consistency was good. Conclusion: There are obvious differences between early lung cancer and small pulmonary nodules in the appearance of spicule sign, fuzzy boundary, angio-assemblage sign, pleural indentation sign and lobulation sign, MSCT has high diagnostic value for early lung cancer and small pulmonary nodules.
