Abstract Objective:To investigate the effect of using modified ultrafihration and balanced ultrafiltration on postoperative pulmonary function of patients with cardiac valvular replacement under extracorporeal circulation. Method:60 cases with heart valvular replacement were randomly divided into the control group(C),conventional ultrafiltration group( D1)and balanced ultrafiltration with modified ultrafiltration group(D2).The hematocrit(Hct),pulmonary function(OI、Cstat and P(A-a)O2),and the concentrations of tumor necrosis factor-α(TNF-α) Endothelin-1(EF-1) and interleukin-6(IL-6)in the three groups were respectively monitored at specific time points.Result:In group( D1,D2) at T2,T3,T4 and T5 point OI was significantly higher than that in control group C(P<0.05).In group D2 P(A-a)O2 at T3, T4 and T5 point was lower than in group( D1,C)(P<0.05).Cstat in group D2 was obviously higher than that of group(D1、C).Hct of ultrafiltration in group(D1,D2) was significantly higher than that in the control group C(P<0.05).The concentration of EF-1,TNF alpha and IL-6 in group D2 at T2,T3,T4,T5 point was lower significantly than the group(D1,C)(P<0.05).Conclusion:Composite ultrafiltration can well improve the postoperative pulmonary function in patients with valvular replacement, reduced the concentration of EF-1 and inflammatory medium in the body.
