Abstract Objective:To summarize the MRI features of enlarged perivascular spaces for improvement of diagnostic rate. Method:90 EPVS patients with MRI indication were selected randomly, their MRI features in respect of shape, size, position, boundary, signal and space occupying effect analyzed. Result:Multiple EPVS were visible bilaterally for all the 90 cases and there had no remarkable difference between the right and left side. The EPVS feature round, oval, linear or tubular shapes with clear boundary and the same direction as the perforator vessels. Their signals were the same as those of the cerebrospinal fluid on T1WI,T2WI and FLAIR sequences( low signal for T1WI,high signal for T2WI and low signal for FLAIR) without the enhancement effect of contrast agent and the space occupying effect. The EPVS occur at the basal ganglia region, the centrum semiovale and subcortex, the pontine, the cerebellum, the thalamencephalon and the periventricle, and their amount increases along with the age.Rank 1 was common in EPVS rating and no obvious change was found in the follow-up cases. Conclusion:The EPVS amount increases along with the age, they mainly occur at the basal ganglia region as well as the centrum semiovale and subcortex in round, oval, linear or tubular shape with clear boundary, and their signals are the same as those of the cerebrospinal fluid on MRI sequences.
